Oesophageal pressure measurements in ventilated preterm babies

چکیده
Sir, Drs Greenough and Morley' conclude that very little ventilator pressure is transmitted to the oesophageal balloon in paralysed babies because of low lung compliance, and support this by showing lack of transmission to chest drains inserted in 2 babies with pneumothorax. Although this fits well with their other observations, they did not say whether there was any intrapleural air remaining on the chest x-ray film at the time of recording. The compliance of an isolated sealed pocket of air under pressure, say in the mediastinum, could be lower than either lungs or chest wall and could provide a significant artefact in their results.

Oesophageal pressure measurements in ventilated preterm babies.
Oesophageal pressure measured with an air-filled, thin latex balloon on a 6 French gauge catheter can accurately measure intraoesophageal pressures in ventilated preterm babies. Intraoesophageal pressures and intrapleural pressures are equivalent. Less than 10% of the applied positive ventilator pressure is transmitted to the oesophageal balloon and the intrapleural catheter (intercostal drain)...
متن کاملGastro-oesophageal reflux in mechanically ventilated preterm infants.
Long duration oesophageal pH recordings were performed on 42 mechanically ventilated and parenterally fed preterm infants for a mean (SD) of 94 (28) hours. Their mean (SD) gestational age was 31.5 (2.6) weeks and birth weight 1514 (448) g. Their mean postnatal age was 19 (10) hours at the onset of pH recording, which was performed with the babies supine. The mean hourly values for the total num...
متن کاملDoes positive pressure ventilation increase arginine vasopressin in preterm neonates?
AIM To examine the effect of intermittent positive pressure ventilation (IPPV) on plasma arginine vasopressin concentration (pAVP) in preterm neonates. METHODS Thirty five neonates were classified, at the time of blood sampling, into three groups: unstable ventilated; stable ventilated; and stable non-ventilated. A modification of an extraction method for pAVP was developed for use in studies...
متن کاملUpper airways obstruction and apnoea in preterm babies.
Measurements of tidal flow, volume, and oesophageal pressure were recorded on a group of 8 severely preterm infants with periodic respiration. Analysis of the flow trace for evidence of cardiac artefact, and the tidal volume trace to identify at what point of the tidal cycle apnoea starts, indicated that upper airways obstruction was associated with about half these attacks. It is surmised that...
متن کاملLow birthweight babies with oesophageal atresia or tracheo-oesophageal fistula.
Of 142 patients of known gestation with oesophageal atresia or tracheo-oesophageal fistula, 21% were preterm with a mean weight of 2 kg, 19% were small-for-dates with a mean weight of 2.23 kg, and the remaining 60% were born at term and weighed more than 2.5 kg. There were almost five times as many preterm and almost eight times as many small-for-date infants as in a normal population. Respirat...
